ASAP Job responsibilities Support safe transitions of care, including handoffs and care plan updates. Perform focused reassessments and escalate concerns through appropriate clinical channels. Work 36 hours per week with 12-hour shifts. Coordinate patient care activities with physicians and other clinical team members.
