Type:

Four-Month Contract with possibility of an extension

Schedule:

Monday through Friday, 8:00 AM - 4:30 PM

Pay:

$55 to $59/hr

Location:

Spindale, NC 28160 Mileage reimbursement provided between patient visits

Responsibilities:

• Provide skilled Physical Therapy services in the home health setting
• Perform admissions, evaluations, treatments, and 30-day reassessments
• Complete resumption of care and recertification visits as needed
• Maintain accurate and timely patient documentation
• Travel throughout assigned coverage areas across 5 counties
• Collaborate with interdisciplinary healthcare teams to support patient care goals

Qualifications:

• MPT or DPT required
• Active BLS certification required
• Previous Physical Therapy experience required
• Home health experience with Oasis documentation preferred
