Pay:

$55 to $59 per hour

Type:

Three-Month Contract, possibility of extension

Schedule:

Monday to Friday, 8AM to 4:30PM

Weekly Hour Guarantee:

32 hours

Primary Responsibilities:

• Provide direct physical therapy services in a home health setting
• Evaluate patients and develop individualized treatment plans
• Perform in-home patient visits across assigned coverage areas
• Document care accurately and maintainpliance with agency standards
• Coordinate care with interdisciplinary team members
• Monitor patient progress and adjust treatment plans as needed
• Ensure high-quality, patient-centered care in the home setting

Qualifications:

• Active Physical Therapist (PT) license required
• Home health experience preferred

Pay Details:
