Job Description

Degree in Physical Therapy, active Physical Therapist license and 1+ year of Physical Therapy experience required. Applicants who do not meet these qualifications will not be considered. Schedule/Hours:
36 hours guaranteed per week
Key Qualifications:
Valid state licensure as a Physical Therapist (PT) Minimum of one year previous experience in a hospital or skilled nursing setting preferred Strong patient assessment and care planning abilities Excellent documentation and communication skills Adaptable, with a positive attitude toward team collaboration and patient care BLS certification
Primary Responsibilities:
Conduct thorough physical therapy evaluations and establish individualized treatment plans Administer therapeutic exercises and interventions tailored to patient needs Collaborate closely with nursing, physician, and rehabilitation staff to ensure integrated care Monitor patient progress and adjust therapies as needed for optimal outcomes Maintain up-to-date and accurate patient records in compliance with regulatory standards Educate patients and families about at-home exercises and injury prevention
