PPV LPN/24/NUR166
We are seeking a Travel Licensed Practical Nurse (LPN) to join our home health team supporting patients across Wicomico County. This role focuses on delivering high-quality, patient-centered care in a home setting while ensuring timely and accurate documentation.

The ideal candidate is dependable, detail-oriented, and passionate about providing compassionate care in a home health environment.

This job will have the following responsibilities:
  • Deliver skilled nursing care to patients in their homes in accordance with care plans and physician instructions
  • Monitor patient health status, including vital signs and symptom assessment
  • Administer medications and treatments within the LPN scope of practice
  • Provide education to patients and caregivers on treatment plans and health management
  • Complete accurate and timely documentation of patient visits, treatments, and outcomes
  • Maintain up-to-date patient records in compliance with organizational and regulatory standards
  • Ensure all care provided aligns with company policies and clinical guidelines
  • Collaborate with physicians, RNs, and interdisciplinary care teams to support patient care plans
  • Communicate changes in patient condition promptly to appropriate stakeholders
  • Participate in care planning and follow-up activities
  • Ensure delivery of high-quality, compassionate care that supports positive patient outcomes
  • Maintain patient confidentiality and dignity at all times
  • Ad to infection control and safety protocols
Qualifications & Requirements:
  • Prior experience in home health care
  • Ability to work independently and manage patient visits efficiently
  • Strong organizational and time management skills
