Requirements:
Current and unencumbered RN licensure. At least 1 year of recent experience in Case Management as an RN. Strong clinical assessment and critical thinking skills. Excellent organizational and communication capabilities. Ability to adapt quickly to new processes and changing environments.
Responsibilities:
Coordinate and manage patient care across the continuum, from admission through discharge. Collaborate closely with physicians, therapists, and external agencies to ensure appropriate care transitions. Perform patient assessments, develop individualized discharge plans, and facilitate resource referrals. Advocate for patient needs and support families through complex healthcare decisions. Maintain thorough documentation in compliance with facility policies and regulations.
