Home Health/Case Manager Job Summary:

We are seeking a compassionate and experienced Registered Nurse to join our team as a Travel RN focusing on Home Health and Case Management. In this role, you will be responsible for providing high-quality care to patients in their homes, assessing their needs and coordinating their care plans.

Responsibilities and Duties:

Conducting comprehensive assessments of patients in their homes Developing individualized care plans based on assessment findings Implementing and overseeing medical treatments and interventions Communicating with physicians and other healthcare professionals to coordinate patient care Educating patients and their families on health management techniques Documenting patient progress and updating care plans as needed Providing emotional support to patients and families during challenging times Adhering to all legal and ethical standards in healthcare delivery

Qualifications and Skills:
