Role Overview:
This nursing position focuses on providing compassionate care to patients in rehabilitation and long-term care settings. You'll collaborate with multi-disciplinary teams, expanding your knowledge across a range of therapeutic and chronic care situations.
Core Responsibilities:
Deliver holistic, patient-centered care that supports recovery and maximizes independence. Coordinate and implement personalized care plans alongside therapists, physicians, and care managers. Monitor patient progress and communicate changes to the healthcare team and families. Administer medications and treatments per established protocols and physician orders. Document all aspects of care with accuracy and attention to legal and facility standards. Support comfort, dignity, and autonomy for every resident.
Desired Qualifications:
Active and unencumbered RN license. Recent clinical experience in a Rehab or Long-Term Care (LTC) environment. Solid clinical assessment, organizational, and communication skills.
