Desired Experience & Qualifications:
Active, unrestricted RN license Minimum of 1 year recent experience in Rehab or Long-Term Care nursing Proficiency in electronic medical record systems Excellent clinical assessment and patient care skills Strong communication and team collaboration abilities BLS certification (current)
Primary Responsibilities:
Deliver skilled nursing care for patients in rehab and long-term care settings Collaborate with the interdisciplinary healthcare team to develop, implement, and evaluate individualized care plans Administer medications, monitor patient progress, and document findings accurately Educate patients and families about care plans, conditions, and expectations Ensure compliance with clinical protocols, safety, and regulatory standards Participate in quality initiatives and continuous improvement efforts
