Travel assignment (varied shifts based on facility need)Qualifications:
Active, unencumbered RN license Minimum of 2 years' experience in Case Management (Acute Care setting preferred) Strong assessment, organizational, and communication skills Ability to manage complex patient cases and collaborate effectively with interdisciplinary teams Current BLS certification requiredResponsibilities:
Provide comprehensive case management services, including patient assessment, care planning, and coordination Advocate for patient needs and facilitate smooth transitions through the continuum of care Collaborate with physicians, social workers, and ancillary staff to ensure optimal patient outcomes Maintain accurate documentation and compliance with hospital policies and regulatory requirementsBenefits & Perks:
