Qualifications:

Active, unencumbered RN license Minimum of 2 years' experience in Case Management (Acute Care setting preferred) Strong assessment, organizational, and communication skills Ability to manage complex patient cases and collaborate effectively with interdisciplinary teams Current BLS certification required

Responsibilities:
