$2,252.50-$2,727/week (40 hours) About the Role We are seeking an experienced Registered Nurse Case Manager for an immediate opening at Providence St. Joseph Hospital Eureka in Eureka, CA. In this role, you will manage a caseload of up to 25 patients across adolescent, adult, pediatric, and geriatric populations, overseeing care coordination, discharge planning, appeals and denials, and utilization review in a fast-paced acute hospital environment. You will work closely with social services and the broader interdisciplinary team to ensure seamless transitions of care and positive patient outcomes. Experience with advance directives, POLST, and protective services processes is required. First-time travelers are welcome to apply with manager approval if experience requirements are met. Locals are accepted. Free parking provided on the first day, with assigned parking given during orientation. Acute care hospital case management experience must be current within the last year — this is a hard stop. Do not apply without meeting this requirement. Requirements Active California RN license (must be currently active) Minimum 2 years of acute hospital Case Management experience required (hard stop — must be current within the last year) Experience with care coordination and discharge planning in a fast-paced acute care environment required Experience with HMOs, IPAs, and managed care organizations required Experience with advance directives, POLST, and protective services processes required Current BLS certification required Epic EMR experience required Travel experience required (first-timers may be screened with manager approval) All RTO must be included at time of submittal Every other weekend availability required Skills & Competencies Care coordination, discharge planning, and needs assessment Appeals and denials management Benefits eligibility verification and prior authorizations DME ordering and arrangement, SNF placement, and transport coordination Advance directive, POLST, and protective services process management Medical necessity determination and utilization review Knowledge of CMS, DRG, HIPAA, and Joint Commission standards Acute hospital experience across ED, ICU, Med/Surg, Pediatrics, and SDU/PCU/IMC/Obs settings Epic EMR documentation proficiency required
