Schedule & Hours:

13-week contract Day shifts, 10 hours (8:00 am - 6:00 pm) Shift options: Thurs/Fri/Sat/Sun or Sat/Sun/Mon/Tues 40 guaranteed hours per week (except during orientation week and holidays) No on-call requirements

Required Experience & Qualifications:

Active WI RN license & American Heart BLS 1-2 years of recent home health and hospice nursing (admissions, symptom management, end-of-life care) Proficiency with Oasis and Epic charting platforms Valid driver's license; comfort with significant daily travel (50-100 miles) Strong time management, resilience, adaptability, and independence Ability to manage documentation efficiently and deliver care in diverse home settings

Primary Responsibilities:

Provide high-quality, compassionate care to adults with chronic and complex conditions in their homes Conduct home health admissions, symptom management, hospice visits (including end-of-life care) Collaborate with interdisciplinary care teams and ensure timely, thorough documentation Promote patient education, wellness, and safety during all visits
