Schedule/Hours 40 guaranteed hours per week 10-hour day shifts (8:00-18:00) Rotating blocks: Thurs-Sun or Sat-Tues No on-call/call back required Key Qualifications Active WI RN license BLS (American Heart Association) certification Valid driver's license Minimum 1-2 years home health and hospice RN experience Recent OASIS proficiency Experience with Epic charting system Skillful in wound care, IV administration, chronic disease management (CHF, COPD, Diabetes, Cancer, Stroke) Able to independently manage a 1:1 adult patient ratio Physically able to drive, manage stairs/uneven landscapes, and work in varied home environments Responsibilities Conduct home admissions, death visits, hospice symptom management Provide compassionate care to adults 18+ managing chronic and terminal conditions Ensure accurate, timely documentation in line with OASIS and Epic requirements Demonstrate adaptability and resilience amidst frequent schedule or case changes Maintain professional appearance (plain, well-maintained scrubs) Comply with current
