Job description Job Description Allegiance Home Health is seeking two detail-oriented and organized Home Health Intake Schedulers to manage the intake process for new patients. This position plays a vital role in ensuring a seamless transition of care for our clients. Key Responsibilities Schedule initial evaluations and assessments for incoming patients New Patient Intake Processing Collect and verify accurate patient information, including demographics and insurance details Communicate effectively with patients, families, and healthcare providers to coordinate care Maintain accurate records and documentation in our electronic health record system Collaborate with interdisciplinary teams to optimize patient intake processes Provide exceptional customer service and support to patients and families

Qualifications:

Previous experience in healthcare scheduling or intake is preferred.
