Job Summary:

Virtua Health is seeking an experienced Clinical Care Manager to support high-quality home health patient care through clinical oversight, documentation review, staff education, and interdisciplinary collaboration. This role serves as a resource to clinicians by promoting best practices in care planning, regulatory compliance, quality improvement, and efficient care coordination. Responsibilities include reviewing and improving clinical documentation and care plans, educating clinicians during orientation and onboarding, conducting recertification reviews, and partnering with operational and IT teams to optimize workflows and Epic care plan functionality.

Qualifications:
