Job Description

JOB TITLE
Home Health Visit Nurse (RN or LPN) Active Ohio LPN or RN in good standing is required At least 1 year of paid experience at a home health agency (or equivalent) is required ; 2-5 years' experience is preferred. 1+ years of experience in the following is highly desirable: Case Management, wound care, PICC/PORT/IV Care, lab draws, plus OASIS and Non-OASIS assessments. Nurse Supervision experience is a plus.
SCHEDULE TYPE & SERVICE AREA
Northeast Ohio (2 options) Required schedule availability: Monday
  • Friday (weekdays) from 8am
  • 5/6pm.
Part-Time RNCM requires a minimum travel radius of Mahoning and Trumbull Counties plus surrounding areas up to 60-minutes (up to 90-minutes on occasion). Full-time RNCM region includes Mahoning, Trumbull, Stark, Ashtabula, Columbiana, Portage, and Carroll counties and areas up to a 2-hour travel radius. Additional cases will be added if more hours are desired (not required). Qualified applicants may reside in or near any primary service area so long as travel requirements are fulfilled throughout the assigned region.

To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health LPN or RN to join our agency as a Home Health Visit Nurse (RN or LPN) to care for clients in and around Trumbull, Mahoning, and Stark counties in OH. This role may be filled as Part-Time only or start as Part-time and quickly build up to Full-time basis , with the option to add additional clients in surrounding areas for additional hours if desired (not required).

  • $32 per hour
RNs:
$35
  • $40 per hour Compensation is based on employee qualifications and certifications, experience, skill specializations, positive employment history, and overall client & team fit.

Qualifications Graduate of an accredited school of professional nursing. Active LPN or RN, with current license to practice as a Licensed Practical Nurse/Registered Nurse in the state of Ohio, is required. At least 1 year in home health industry + 1 year of nursing experience. Additional Home Health Nursing experience/ familiarity is highly desired. Active CPR and First Aid certificates highly desired (for speed to start care), but agency will provide recertification if required. Minimum of 1 year direct patient experience with vent care is required. Good interpersonal communication skills

Willingness and ability to pass state and federal background check. Position Summary A successful Visit Nurse at Home Care Network, Inc., performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and agency. All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN/RN delivers care to clients as delegated by the RN Case Manager.

Clinical Administrator/Director of Nursing Essential Functions/Areas of Accountability:
Provides professional nursing care under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per agency guidelines. Provides skilled nursing visits for clients as directed by the RN Case Manager. Follows the care plan established by the Registered Nurse. Recognizes and reports changes in client condition to the Case Manager and physician, as directed. Communicates information on a timely basis, consistent with agency policy and urgency of the situation. Communication is verbal, as needed, and through complete and accurate documentation in the clinical record. Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Case Manager. Individuals with the license designation of "
LPN M-IV
" may perform venipuncture for the purpose of obtaining blood samples for laboratory analysis. Promotes personal safety and a safe environment for clients and coworkers. Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures. Recognizes and responds appropriately to potentially unsafe situations. Demonstrates safe practice in the use of equipment. Makes recommendations on improvement of safety. Performs job in compliance with agency policies and procedures and professional and community standards. Accepts responsibility for professional nursing role. Follows applicable state and federal laws and agency standards. Attends all meetings as required. Maintains confidentiality in all aspects of the job. Refrains from discussing client or staff identities, conditions, or circumstances in public or with others who do not have a legitimate interest in the person. Participates in the development, implementation, and evaluation of the Agency Quality Improvement Program and pertinent activities. Performs other related duties and responsibilities as deemed necessary.
