We are looking for a dependable team player with a passion for top quality care to serve the residents of our Assisted Living and Memory Care community .
Essential Functions, Duties, and Responsibilities:
Tasks may include, but are not limited to the following: 1. Ability to perform physical assessment of resident and identifies problems related to physiological, psychological, environmental and social factors. 2. Ensure quality and safe delivery of home health services to residents. 3. Collect and analyze pertinent health related information. 4. Obtain physician's order prior to initiating treatments or changes in treatments. 5. Implement plan of care formulated by physician and nursing team. 6. Utilize safe and appropriate techniques/nursing skills in providing resident care. 7. Report pertinent changes in patient's condition to the nurse director and/or physician. 8. Report changes in patient status to the office such as hospitalization, discharges, and physician appointments. 9. Provide accurate and timely documentation consistent with plan of care. 10. Assess and provides resident and family education and information pertinent to diagnosis of plan of care. 11. Communicate with patient and/or families feedback from physicians. 12. Follow infection control and universal precautions when providing patient care. 13. Communicate directly with other disciplines involved in resident care on a regular basis, including weekends. 14. Participate in team meetings to focus on resident problems. 15. Administer medication per physician orders. Observe for any adverse reaction and notify supervisor and/or physician accordingly. 16. Maintain safe and healthy work environment and use safe work methods and procedures in accordance with company clinical standards. 17. Assist resident in learning appropriate self-care activities. 18. Organize and monitor human and material resources necessary for meeting needs of resident. 19. Recognize emergencies and life-threatening situations and initiate appropriate actions as established by the agency. 20. Document all information in a timely and accurate manner. 21. Follow all appropriate safety rules, guidelines and precautions. 22. Maintains confidentiality and resident rights, regarding all resident personal information. 23. Contact office with any changes in visit schedules. 24. Participate in staff development and training activities. 25. Cooperates with co-workers, including assisting with Orientation. 26. Follow dress code demonstrating appropriate appearance and personal hygiene. 27. Provide proper notification and advance notice for absences or tardiness. 28. May serve as shift supervisor as need dictates, and acts as clinical lead in absence of Director of Nursing.Qualifications- Education, Experience, and Skills:
1. Graduated from an accredited Nursing program and currently holds active LPN or RN license in WA State. 2. Has had at least one year of nursing experience in a health care facility. 3. Has had experience in geriatric nursing, emergency care, first-aid. 4. Must obtain valid Continuing Education credits as required by the State. 5. Desirable to have training in the special needs of the cognitively impaired resident i.e, Alzheimer and Dementia training with focus on Validation Therapy. 6. Must be able to cope with the mental and emotional stress of the position. 7. Must possess the ability to work effectively with physicians, management, employees and other outside contacts. 8. Possess the ability to make independent decisions when circumstances warrant such action. 9. Possess leadership ability and the willingness to work with and supervise staff.Associated Knowledge, Skills and Abilities:
Possesses knowledge and experience in performing treatments and utilizing appropriate infection control techniques. Knowledgeable about current treatment protocols and documentation requirements. Must display genuine interest in seniors and compassion for the aging; must possess an openness in maintaining a multi-disciplined team approach to health care of the elderly; must possess ability to maintain/strengthen good working relationship with staff, residents, families, physicians, and wound specialist; must possess good skills in planning, organizing, directing and managing all work assignments. Must possess a good sense of efficiency and in successfully completing all work assignments; must be knowledgeable and supportive of the Community's philosophy and objectives; must be flexible and open to change; must be knowledgeable of current regulatory standards; may be required to work extra hours and/or be on call during off hours.
