Position Overview:

We are hiring a full-time Nurse Practitioner with wound care experience and active PECOS Certification to perform in-home or facility-based visits throughout our service areas. The ideal candidate will be an independent provider with a strong clinical background and the ability to manage complex wounds in coordination with our care team. Job Duties and Responsibilities Evaluate and manage the extent of wounds and develop individualized care plans at home or facility settings in collaboration with patients and care teams Perform debridement, dressing selection, and wound care procedures per best practices. Maintain compliance with HIPAA regulations while documenting patient visit information accurately/ timely in our EMR system Travel to patient locations within a 60-mile radius of your home base Communicate with multidisciplinary teams, including physicians and nursing staff as needed Ensure compliance with CMS, state, and practice-specific regulatory requirements Provide education to patients and families regarding wound care techniques, infection prevention, and overall health maintenance as needed Monitor and evaluate patient responses to treatments, adjusting care plans as needed Engage in continuous professional development to stay updated on the latest advancements in wound care and infectious disease management. Qualifications Current and unrestricted Nurse Practitioner License in Texas (Required) Active PECOS enrollment (Required) Wound Care Certification or Clinical Wound Experience highly preferred Valid driver's license and reliable transportation Strong ability to work independently and adapt to mobile care settings Excellent documentation, time management, and communication skills
