Home Visit Coordinator Albuquerque, NM Pay:
$18.00 - $20.00 per hour

Monday-Friday Position Summary El Mirador Home Care is seeking a dependable, organized, and compassionate PCS Administrative Assistant / Home Visit Coordinator to join our Albuquerque team. This position combines front office administrative responsibilities with client home visit support and coordination activities. The ideal candidate will be comfortable working in a fast-paced office environment while also assisting with client home visits, documentation, and compliance-related activities in the field. This is an excellent opportunity for someone interested in healthcare administration, case coordination, or home care services. Essential Duties and ResponsibilitiesAdministrative Support Provide front desk coverage and greet clients, caregivers, and visitors professionally. Answer incoming phone calls and direct inquiries appropriately. Assist with scheduling client appointments, home visits, and caregiver assignments. Maintain accurate client and employee records. Scan, upload, file, and organize electronic documentation. Assist with data entry and general office support functions. Monitor and follow up on required documentation and compliance items. Home Visit Coordination Assist supervisors with client home visits as needed. Complete required visit documentation and reports. Support client assessments and service reviews. Follow up with clients regarding services, concerns, and scheduling needs. Coordinate communication between clients, caregivers, supervisors, and healthcare professionals. Compliance and Operations Support Assist with tracking employee compliance requirements. Support EVV/Authenticare documentation and follow-up activities. Assist with incident reporting and corrective action documentation. Prepare records for audits, reviews, and quality assurance activities. Support intake, staffing, and scheduling processes. Qualifications High School Diploma or GED required. Bilingual English/Spanish preferred. Previous administrative, customer service, healthcare, or home care experience preferred. Strong communication and organizational skills. Proficiency with Microsoft Office, Outlook, and basic computer applications. Ability to manage multiple tasks and deadlines. Reliable transportation, valid driver's license, and current automobile insurance required. Ability to travel locally throughout the Albuquerque area for client visits and agency business. Preferred Experience Home care, healthcare, Medicaid, or PCS experience. Scheduling or staffing experience. Experience working with electronic medical records (EMR/EHR) systems. Experience working with seniors, individuals with disabilities, or community-based services.
