Job Description
This position will be responsible for the end-to-end coordination, processing and monitoring of extended foster care, postsecondary education services and support and aftercare eligibility documentation and payments for young adults (agers 18-23).
ESSENTIAL FUNCTIONS
- Establish and maintain effective, positive working relationships with HFC internal and external customers
- Review applications for completeness, accuracy, and statutory eligibility
- Monitor and verify approvals, terminations and documentation in all systems
- Maintain and track assessments and housing plans
- Identify missing or incorrect documentation and communicate deficiencies
- Monitor young adult age milestones and notify CMOs of upcoming age-outs
- Collect and review W‑9s, new provider forms, and required documentation for young adults, care-lords and landlords
- Submit documentation for provider setup and ensure accuracy of provider ID
- Maintain and track shared living agreements and ensure documents align with eligibility and payment rules
- Prepare and finalize monthly payment checklist with correct codes and provider identification, with submission to the Finance team timely
- Review financial reports to ensure payment accuracy and resolve discrepancies prior to release
- Serve as the primary liaison to CMOs, life coaches, and internal/external customers for documentation accuracy
- Maintain audit-ready records for eligibility determinations, approvals, provider documents, checklists and payment authorizations
- Maintain knowledge of statues, processes and procedures to update policies, forms and/or checklists
- Recommend administrative workflow improvements to reduce errors, rework and process delays
- Performs other unit duties as assigned High school diploma or GED required.
