Job Description

Flagler Cares is looking for a compassionate and highly organized Intake & Eligibility Coordinator to serve in a multifaceted role in supporting the mission of Flagler Cares and Volusia Cares to provide accessible, compassionate, and coordinated care to community members in need. This position combines two primary areas of responsibility, intake coordination and eligibility assistance. Each is essential to ensure that individuals and families receive timely, effective support and connections to appropriate services. Most Essential Work:
Client engagement and needs assessment Review and organization of eligibility documentation based on program criteria What will you be doing? Serve as initial point of contact for individuals seeking assistance Provide direct, high-quality support to clients applying for emergency rent and mortgage assistance Assist with screening applications, collecting necessary documentation, and making an eligibility determination. Referring clients to local services and assisting with resource navigation Using Motivational Interviewing techniques to empower client decision-making Maintaining accurate documentation using our referral system (LINC) Are you our next Intake & Eligibility Coordinator? Associate's degree in a human services field is highly desirable but significant experience can be considered as a substitute along with a High School Diploma. Minimum two years of experience providing client services, case management or eligibility determination assistance. Experience conducting benefits screenings is preferred. Excellent communication skills with the ability to manage multiple tasks simultaneously, prioritize competing demands, and work with a strong sense of urgency in a fast-paced environment. Commitment to upholding professional and ethical standards. Successfully pass a Level 2 Background Screening. Current Florida Driver's License. Paid Time Off (PTO) accrual beginning on your first day of employment, earning 130 hours in your first year of employment.
