Job Description

I. Summary of the main function/purpose of the position: The Eligibility Coordinator/Resource ID Specialist works collaboratively with newly diagnosed or newly identified HIV positive clients and assists in the intake process into case management services at BASIC. The position is responsible for collecting Ryan White and HOPWA related data, completing the financial eligibility process, and assembling eligibility paperwork prior to assigning a client to a case manager. This position serves as significant clerical and administrative support to the Client Services department in the general areas of finance, billing, and grant reporting. II.
Principal duties/responsibilities:
  • Collect and maintain information on available housing and community resources for those living within Area 2A.
  • Updates BASIC Community Resource Booklet monthly and provides updated documentation to Part
B/ HOPWA
Data Clerk by the end of each month.
  • Maintains current active client rosters for Ryan White Part B and ensures appropriate storage/ disposal of closed/ inactive client files as directed by supervisor.
  • Compile and maintain demographics and eligibility logs for Ryan White Part B/HOPWA clients, submit to Part B Data Clerk by the end of each month
  • Maintains all required intake information for client files in a secure and safe location.
  • Assist clients with completing necessary forms.
  • Builds and maintains professional trusting relationships with clients and providers.
  • Maintains an updated inventory roster of providers for the department and provides other resources in Area 2A to clients.
  • Collect each transportation and copy log, enter the calculations in appropriate forms and submit them to Executive Director by 3rd of each month.
  • Track and distribute the trolley passes and gift cards as assigned.
  • Completes checklist for the client closet and submit to Executive Director quarterly
  • File all processed Ryan White Part B purchase orders, complete HOPWA documentation by the end of each month.
  • Maintain the inventory log for all equipment at BASIC, provide inventory list to Executive Director by the end of each quarter.
  • Maintains the closed and ineligible for services client files, conduct routine purge of files as needed.
  • Record pertinent information accurately, conducts eligibility screenings as required.
  • Pick up trolley passes, track and distribute to clients with provided documentation as needed
  • Track and distribute gift cards to clients quarterly
  • Upload all necessary documentation and complete task in CAREWare for incoming clients within 5 days
  • Assign incoming clients a case manager and Unique ID, as needed
  • Prepare the initial chart for clients and subsequent charts, as needed
  • Ensure clients schedule an appointment with Linkage Coordinator and Peer Navigator
  • Maintain neat, tidy clutter-free workspace.
  • Monitor the lobby area and front restrooms, be sure the glass window, floors, counters, information shelves are always clean and organized.
  • Respect client privacy and confidentiality.
  • Updated staff mailboxes, as needed
  • Maintain acceptable coverage for the front desk
  • Maintain materials in front lobby, be sure documents, pamphlet, brochure, etc. are up to date. Discard outdated material at the discretion of the Director of Client Services.
  • Monitor the front lobby doors, be sure the doors are closed and locked at all times.
  • All other duties assigned III. Occasional duties or projects which may be performed at irregular intervals:
  • Assists with community activities designed to raise funds for or awareness of BASIC.
  • Other related duties, as required.

Educational and/or experience requirements:
High school diploma or GED Experience with accounting functions, word processing, professional communication. Requires the ability to attend to several projects, respond to the needs of multiple staff and manage general office systems of a non-profit agency. VIII.
Certification or licensing requirements:
Valid Driver License Insured Vehicle IX. Specialized equipment or machines used in the course of the duties of the position. Personal Computer, printer, calculator, copier and other standard office equipment and machines. X.
