You Become the Bridge Between Referral and Care When a new referral comes in, there's a lot that needs to happen before care can begin. You'll help determine whether we have everything needed to move forward. You'll review the referral. Confirm eligibility. Gather missing documentation. Communicate with hospitals, clinics, physicians, facilities, clients, and families. Enter information accurately into the EMR. And help make sure the clinical team has the information they need to take over from there. When intake is done well, the entire admission process becomes easier for the patient, the family, the referral source, and the clinical team . What You'll Be Doing Review and screen incoming home health referrals. Determine whether referrals meet admission criteria. Verify payer, insurance, and reimbursement eligibility . Contact referral sources to obtain missing medical documentation. Conduct intake conversations with clients and families. Enter accurate and timely patient information into the electronic medical record. Review physician orders, diagnoses, insurance information, and supporting clinical documentation. Communicate with hospitals, clinics, physician offices, senior living communities, and other referral partners. Coordinate with internal clinical and operational teams to help move referrals toward admission. Track referral status and follow up on outstanding documentation or information.
