NOW HIRING INTAKE LIAISON!
As an Intake Liaison for our PACE (Program of All-Inclusive Care for the Elderly) program, you'll play a vital role in helping prospective participants and their families navigate the enrollment process with compassion, professionalism, and outstanding customer service. If you're a people person who enjoys building relationships, coordinating details, and making a meaningful impact, we'd love to meet you! As our Intake Liaison, you'll serve as the first point of contact for prospective participants, families, referral sources, and healthcare partners. You'll help ensure every individual receives a welcoming, informative, and seamless enrollment experience.
Responsibilities include:
Responding to incoming referrals and inquiries about the PACE program Building relationships with participants, families, physicians, hospitals, and community partners Scheduling assessments, home visits, tours, and enrollment appointments Coordinating the enrollment process from referral through admission Preparing enrollment packets and maintaining accurate documentation Requesting and organizing medical records and required paperwork Maintaining participant information within our Electronic Health Record and CRM systems Collaborating with the interdisciplinary team to ensure a smooth transition into the program Providing exceptional customer service while maintaining confidentiality and professionalism Assisting with reports, referral tracking, and enrollment metrics
Requirements:
Required Associate degree in Healthcare Administration, Business Administration, Human Services, or a related field; or equivalent combination of education and experience. Minimum of two (2) years of customer service, healthcare, admissions, intake, or enrollment experience. Excellent interpersonal, written, and verbal communication skills. Strong organizational and time management abilities. Proficiency with Microsoft Office (Word, Excel, Outlook). Ability to manage multiple priorities in a fast-paced environment. Ability to maintain confidentiality and professionalism. Preferred Previous experience in PACE, senior healthcare, home health, hospice, managed care, or long-term care. Experience with Electronic Health Records (EHR) and Customer Relationship Management (CRM) systems. Knowledge of Medicare, Medicaid, and healthcare eligibility requirements. Experience working with older adults and their families. Physical Requirements This position primarily works in an office environment and requires prolonged sitting, computer use, telephone communication, and occasional standing, walking, bending, lifting, and carrying up to 20 pounds.
