LVN Intake Specialist Full-Time | Remote We're looking for a detail-driven LVN Intake Specialist to join our team. In this role, you'll manage incoming referrals, ensure documentation is accurate and compliant, and help coordinate a smooth admission process for patients. This is a great opportunity for someone who thrives in a fast-paced environment and wants to play a key role in patient care coordination. What You'll Do Process and enter referrals in HCHB accurately and in a timely manner Review and approve Face-to-Face documentation to ensure Medicare compliance Ensure all referral documentation is complete; coordinate with DONs as needed Run daily reports and resolve any missing or incorrect information Partner with Admission Coordinators to move pending referrals to admission Communicate with physician offices and internal teams to gather required info Maintain continuity by ensuring coverage and handoff of pending referrals Support the Intake team and assist with additional tasks as needed What We're Looking For Active LVN/LPN license in good standing Must live in Norman, OK or surrounding area Experience in home health intake or similar healthcare setting preferred Familiarity with HCHB and/or EMR systems is a plus Knowledge of medical terminology and ICD-10 preferred Strong attention to detail and accuracy Comfortable communicating with physicians and clinical teams Highly organized with the ability to multitask in a fast-paced environment Strong computer skills (typing, systems navigation, documentation) Team-oriented with a positive, flexible attitude Why Join Us Fully remote position with a supportive team Opportunity to play a critical role in patient care coordination Stable, full-time schedule
