- Contract Job Summary We are an established, state-licensed home care agency preparing for our provider enrollment with South Carolina Medicaid (SCDHHS).
We are seeking an experienced, organized, and licensed Registered Nurse (RN) to serve as our credentialed nurse for the enrollment process and to build out our long-term clinical compliance infrastructure. [1, 2]Initially, you will assist in aligning our clinical paperwork, staff competency frameworks, and policy manuals with state standards. Once enrollment is complete, you will transition into conducting initial client assessments and overseeing care plans. Responsibilities Deliver personalized patient care by assessing individual needs and implementing appropriate interventions in accordance with established protocols. Administer medications and treatments accurately, adhering to medical documentation standards and HIPAA confidentiality requirements. Utilize electronic health record (EHR) systems such as Epic, Cerner, or eClinicalWorks to document patient information comprehensively and securely. Assist with patient transfers using safe manual handling techniques and equipment such as Hoyer lifts. Monitor vital signs, perform phlebotomy procedures, and provide specialized care for patients with complex conditions such as dementia, Alzheimer's disease, or those requiring memory or behavioral health support. Collaborate effectively with multidisciplinary teams across emergency medicine, ICU, trauma centers, hospice, pediatrics, geriatrics, and other specialized units to ensure seamless patient care delivery. Maintain compliance with infection control practices including aseptic techniques and safe lifting practices while working with infectious disease cases or in sterile environments. Support patients' physical comfort through catheter care, medication administration, and other nursing procedures while maintaining meticulous medical records. Respond promptly to urgent situations in outpatient or inpatient settings by applying advanced clinical skills in acute care scenarios. Qualifications Current licensure as a registered nurse (RN) or licensed practical nurse (LPN), with valid state certification. Proven experience working in diverse healthcare environments such as hospitals (including trauma centers), long-term care facilities, assisted living communities, or home health agencies. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or eClinicalWorks for accurate medical documentation. Critical care experience preferred—particularly ICU, emergency medicine, or Level I/II trauma center exposure—along with familiarity in hospital medicine and post-acute care settings. Strong knowledge of medical terminology, physiology, and anatomy relevant to patient assessment and treatment planning. Ability to perform heavy lifting safely using proper techniques; experience with manual handling equipment is advantageous. Excellent caregiver communication skills coupled with compassionate patient service approach—especially when working with geriatric patients or individuals with disabilities. Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality at all times. Basic life support (BLS) certification required; additional certifications such as ACLS or PALS are a plus.
