This role focuses on patient connection. You will be assigned to one of our clinics in the DFW area and introduce our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs to eligible senior patients in-person. We need someone who has a natural ability to connect with the senior population, establish immediate trust, and build authentic rapport with patients.
Senior Outreach :
Initiate conversations with senior patients at our clinics to introduce continuous care and remote monitoring programs.
Problem Solving :
Identify pre-existing/enrolled patients who may not be engaging in our CCM and RPM programs and collaborate with them to solve any of their impeding issues to ensure patient participation.
Trust & Rapport Building :
Serve as a friendly, reassuring voice, making seniors feel valued, heard, and comfortable throughout the conversation. Address patient hesitation with empathy, answer questions patiently, and secure verbal consent.
Required Communication Strengths Active Listening :
Ability to listen deeply to senior patients, pick up on subtle cues, and validate their feelings or concerns.
Patience & Empathy :
A naturally calm, unhurried demeanor tailored to the pacing and conversational needs of older adults.
Relationship Driven :
A proven history of building fast, meaningful connections with people from diverse backgrounds.
Clarity :
Ability to explain new ideas clearly without using confusing medical jargon or technical slang.
Reassurance :
A comforting communication style that melts away skepticism and builds immediate credibility.
