Job Description

Job Title:
Patient Enrollment Specialist Position Summary:
The Patient Enrollment Specialist is responsible for identifying, educating, and enrolling eligible patients into care management programs, including but not limited to APCM (Advanced Primary Care Management), RPM (Remote Patient Monitoring), and COCM (Collaborative Care Management). This role serves as a key point of contact for patients, helping them understand program benefits and supporting their ongoing engagement in care services.
Key Responsibilities:
Conduct outbound and inbound patient outreach via phone and other communication channels to introduce care management programs Educate patients on program services, benefits, and eligibility requirements in a clear and compassionate manner Assess patient eligibility and obtain consent for enrollment in programs such as APCM, RPM, and COCM Accurately document patient interactions, consent, and enrollment status in the electronic health record (EHR) or care management platform Collaborate with clinical staff and care teams to ensure smooth onboarding and handoff of enrolled patients Maintain a consistent pipeline of outreach efforts to meet enrollment goals and program targets Address patient questions, concerns, or barriers to participation and escalate as needed Ensure compliance with HIPAA and all regulatory guidelines related to patient communication and enrollment Track and report enrollment activities and outcomes as required
Qualifications:
High school diploma or equivalent required 1+ years of experience in healthcare, patient services, call center, or care coordination preferred Strong communication and interpersonal skills with the ability to build rapport quickly Comfortable discussing healthcare programs and explaining services to patients Experience with EHR systems and/or CRM tools is a plus Ability to work independently and manage time effectively in a fast-paced environment Knowledge of care management programs (RPM, APCM, COCM) preferred but not required

Work Environment:

Pay:
$10.00 - $14.00 per hour
