Job Summary:
The candidate chosen for this position will perform the duties listed below in multiple counties in Georgia. The position is field based, with supervision from the Regional Field Office. Assists people who have been prioritized for a NOW or COMP waiver with the waiver service entry process. Complete required documentation necessary for waiver service entry. Maintain knowledge of local resources for people who have support needs that cannot or should not be funded through the NOW or COMP waivers. Communicates with individuals in a manner appropriate to the individual's abilities to gather required information; communicates with families and other authorized parties to obtain information necessary to maintain awareness of changing circumstances, current level of need, and specific support needs Participates in regional planning list meetings to support other team members and improve knowledge. Assist individuals who are moving into waiver services their families with the Medicaid application process to the degree necessary Assist individuals with IDD to choose service providers by providing information on providers, facilitating meetings with potential providers, or scheduling provider fairs as warranted Develops and maintains knowledge of community resources and maintains a network of community contacts; uses this knowledge to educate and advise families and providers/caregivers, and make necessary referrals for non‐waiver supports Maintains close contact with those on the planning list who have significant health needs and assist these individuals and their families to get all support available through the Medicaid state plan Complies with HIPAA and all DBHDD policies regarding confidentiality of protected health information Reports concerns of abuse, neglect, exploitation, or immediate risk to an individual as required by policy Assist with State Service Coordination tasks as needed Preferred Qualifications ‐External:
Experience with or knowledge of individuals with intellectual/developmental disabilities, Medicaid waivers, andDBHDD I/DD
services; experience working successfully without daily contact with a supervisor. FOR THIS TYPE OF EMPLOYMENT, STATE LAW REQUIRES A NATIONAL AND STATE BACKGROUND CHECK AS A CONDITION OF EMPLOYMENT.
Bachelor's degree in a related field from an accredited college or university and two (2) years of job-related experience; or two (2) years of experience at the lower level Social Svcs Prgm Coord 1 (SSP030) or position equivalent.Note:
