Job Description

Overview The Aging Services Programs at Wright County Community Action (WCCA) provide services for older adults and their caregivers in Wright County.
Our current services include:
Assisted Transportation, Homemaker Services, Caregiver Transportation, and Caregiver Homemaker Services. Our mission is to provide support to older adults and their caregivers by assisting older adults to live in their homes independently as long as possible and supporting informal Caregivers in their role. Through our services, we reduce risk for our clients, improve their quality of life, and provide support as individuals age in place. The Aging Services Coordinator will have responsibility for volunteer recruitment and volunteer management, as well as Caregiver Education, Program Quality Control, providing Homemaker Services, completing Needs Assessments, developing service plans for our Homemaker Clients, Data Management, and Reporting. Education and Experience Bachelor's Degree in social work, nursing, counseling, gerontology, human services, or related degree (or a combination of similar education and experience). Experience overseeing staff or volunteers to ensure quality and consistency. Experience navigating social service programs is preferred. Moderate knowledge and experience of common barriers that seniors face in living independently, and resources that are available. Experience in keeping accurate records and maintaining organized files. Demonstrated strong decision-making skills and resourcefulness. Duties Meet all local, State, and Federal funding reporting requirements. Conduct accurate and appropriate computer-generated record keeping for volunteer activities. Follow the agency's Fiscal protocol when dealing with expenditures and reporting. Ensure that volunteers work in a safe, healthy, and supportive environment in accordance with all appropriate facility guidelines. Receive all incoming calls or emails concerning the Aging Services in a courteous and friendly manner, confronting situations tactfully as needed. For each transportation request, find available ride options per program procedures. Convey accurate program and ride information to and from riders, drivers, and service providers. Act as liaison to other service providers in coordination of services. Accurately document all required data. Support outreach, recruitment & distribution of information for all agency programs & services. Provide Homemaker Services for a limited number of clients and as needed if the homemaker is unavailable. Develop and implement a service plan documentation system for homemaker services. Register New Clients. Home visits at least annually with all clients to do quality control, needs assessments, and update income information. Provide Resources to Older Adults and their Caregivers to assist with unmet needs. Advocate for other WCCA programs and services and communicate referrals to other programs. Assist with facilitating services and maintaining the waitlists for services. Build partnerships with local organizations that serve older adults and their caregivers. Attend local events to educate and raise awareness about Caregiver Services. Develop marketing materials for Caregiver Services at WCCA. Requirements Ability to relate courteously and professionally, displaying a sensitive and caring attitude toward program participants, providers, staff, and volunteers by phone and in person while maintaining confidentiality. Ability to motivate people to understand the need for coordinated transportation service in our area to recruit volunteers and enroll additional clients. Willingness to be assertive, tactful, use good judgment, and work as a team member in the ongoing development of Aging Services Programs. Ability to effectively communicate, both written and verbally, with family members, program staff, and Older Adults. Must demonstrate the ability to use a computer and software such as Microsoft Excel in daily record keeping and routine reporting. Must have access to reliable transportation and a current driver's license. Ability to stand, walk, crouch, etc., to provide household cleaning duties.
Pay:
$20.00 - $22.50 per hour
