Job Description

Unit Clerk Full-time $17/
HR Job Summary:
At Bridgewater Center for Rehabilitation and Nursing, we pride ourselves on providing compassionate and skilled nursing care to our community. We are seeking a dedicated and organized Unit Clerk to join our team, supporting daily operations and ensuring efficient communication and care coordination for our residents.
Responsibilities:
Patient Record Management :
Maintain and update patient records, review charts for accuracy and completeness, and notify the healthcare team of necessary updates.
Resident Transportation :
Coordinate transportation for residents to appointments and activities, accompany residents as needed, and ensure timely arrivals.
Clerical Support :
Transcribe nurses' orders per policy, perform general clerical duties such as faxing, copying, and answering phones, and greet visitors and providers, assisting as needed.
Communication & Coordination :
Communicate with healthcare team members, patients, and visitors via phone and intercom, and update and manage patient profiles, ensuring information is current upon admission, discharge, or transfer.
Support Nursing and Recreation Staff :
Assist nursing staff with bed-making and restocking utility rooms, transport residents to activities, rehabilitation sessions, and appointments as needed, and assist with receiving and sorting supplies.
Inventory & Supply Management :
Stock and manage supplies, including linens and IV supplies, and assist with receiving and sorting supplies, ensuring accurate inventory and charge tracking.
Medical Information System Maintenance :
Prepare and maintain the unit's medical information system, including uploading forms and resident records to the electronic system.
Requirements:
High school diploma or equivalent Reliable transportation to and from the facility Strong work ethic with a commitment to excellence
