Full Job Description About Us:
Bridgeway Services is a Care Management Organization (CMO) that provides oversight for youth adjudicated by the Third Circuit Judicial Court for probation. We are contracted with Wayne County's Department of Juvenile and Youth Services.
General Summary :
This role has two primary responsibilities: coordinating referrals to out-of-home care facilities and ancillary services, including all related documentation and tracking; and managing internal tracking processes to support various utilization reports. This is a hybrid position. Some in office days will be required. The office schedule is Monday through Friday from 8:30am-5pm.
Principal Duties And Responsibilities Include But Are Not Limited To:
Receives and reviews all assessments packets. Determines appropriate residential placement of youth based on assessment, referral information and contracted provider grid. Completes a Provider Referral Information Packet and forwards it to potential providers. Coordinates the transfer of juveniles to out-of-home providers using appropriate transport contractors. Completes Authorization for Services with contracted out-of-home providers upon placement of a juvenile. Completes Length of Stay (LOS) Report for updates on Detention Status. Updates Referral Coordinator Worksheet as needed. Makes referrals to contacted Supplemental Service Providers for youth as requested by Case Manager. Coordinates start date for service and provides Authorization of Service. Tracks completion of treatment plans by providers and follows up as needed. Ensures that all case notes are accurate, timely and refer to placement coordination activities and other pertinent job tasks as appropriate. Updates and maintains provider/program information. Updates client data base Updates detention roster and distributes to partner agencies Handles all Medicaid related issues
Qualifications :
Minimum:
Bachelor's degree in human services or related field Skilled in Microsoft Office Suite with specific knowledge of Excel Excellent written and verbal communication skills to effectively interact with partners and stakeholders Strong organizational and time-management skills to manage multiple tasks Detail-oriented to ensure accuracy Knowledge of Juvenile Justice in Wayne County or Michigan is preferred, but not required. However, preference will be given to candidates who have experience in Child Welfare or Juvenile Justice.
