Job Description

Carlyle Senior Care of Blackville is seeking a compassionate, organized, and motivated Admissions / Social Services Coordinator to join our team! Our facility provides both short-term rehabilitation and long-term care services, and we are committed to delivering exceptional care and support to our residents and families. The Admissions / Social Services Coordinator plays an important role in welcoming new residents, assisting families through the admission process, coordinating discharge planning, and supporting the emotional and social well-being of our residents.
Responsibilities Include:
Coordinate admissions and resident transitions into the facility Communicate with hospitals, referral sources, residents, and family members Assist residents and families with social service needs and discharge planning Maintain accurate admission and social services documentation Help ensure resident rights and quality of life are upheld Build positive relationships with residents, families, and interdisciplinary team members Assist with care planning and resident advocacy
Qualifications:
Previous experience in healthcare admissions, social services, case management, or long-term care preferred Excellent communication and organizational skills Compassionate and resident-centered approach Ability to multitask and work in a fast-paced environment Knowledge of Medicare, Medicaid, and long-term care processes is a plus
