Cuba, NY Position Summary The Admissions Coordinator is responsible for managing the entire intake and admissions process for our 60-bed skilled nursing facility. They serve as the primary liaison between referral sources (hospitals, physicians, discharge planners), potential residents, and their families. This role ensures maximum occupancy by managing inquiries, verifying insurance eligibility, reviewing clinical appropriateness, and guiding families through the transition to care. The Coordinator ensures all regulatory compliance is met while providing empathetic, timely customer service. Key Responsibilities 1. Referral Management & Intake Receive and process inquiries and referrals from hospitals, physicians, and families in a timely manner. Review clinical documentation in collaboration with the SNF Director of Nursing to determine if the facility can meet the patient's needs and approve/decline referrals accordingly. Maintain a current waiting list and manage bed capacity to meet census goals. 2. Financial Verification & Insurance Verify Medicare, Medicaid, Managed Care, and other insurance benefits prior to admission. Obtain necessary authorizations from insurers to ensure revenue integrity. Educate patients and families on financial requirements and obtain necessary signatures. 3. Admission Coordination & Documentation Coordinate room assignments and ensure the room is prepared for the new resident. Complete all admission paperwork efficiently, including admission agreements and face sheets. Ensure all documentation is compliant with facility policies, as well as state/federal regulations. 4. Marketing & Community Liaison Conduct tours of the facility for prospective residents and families. Build and maintain relationships with local hospital discharge planners, social workers, and physicians to promote the facility. 5. Communication & Compliance Act as the initial point of contact, ensuring a welcoming and empathetic environment for new residents and families. Communicate with the interdisciplinary team (Nursing, Social Services, Therapy) to ensure a smooth transition. Maintain accurate, daily census reporting.
Requirements and Qualifications Experience:
Minimum 1-2 years of experience in admissions, case management, or marketing in a skilled nursing or long-term care setting.
Education:
Associate degree preferred; high school diploma required.
Knowledge:
Working knowledge of Medicare/Medicaid regulations, managed care, and medical terminology.
Technical Skills:
Proficiency in MS Office and EMR systems (e.g., Point Click Care).
Interpersonal Skills:
Exceptional communication skills, ability to manage stress, and high empathy. Preferred Skills Experience with hospital discharge planning processes. Proven ability to work in a fast-paced environment.
