Job Description

Admissions RN or Social Worker Direct Report:

Job Summary:
Under the supervision of the Administrator, you are responsible for providing the highest quality of care in our 117-bed facility to our residents in a family-like environment. You will assist in census development and the admission process as well as initiate and implement marketing activities which support census growth and admissions. An Admissions Coordinator in long-term care manages the process of admitting new residents to a facility, including reviewing medical and financial documentation, coordinating with families and healthcare providers, and ensuring compliance with regulations. They also play a role in discharge planning, establishing ownership of oversight, communicating with IDT, Preferred Providers, and be proactive in communicating with each resident post discharge.
Qualifications:
Successful completion of an accredited Social Worker or RN program. Currently licensed as a Social Worker or RN in the Commonwealth of Pennsylvania. BLS/CPR certified. Knowledge of medical terminology and nursing practices. Knowledge of measuring and administering medications. Prior knowledge or experience with Point Click Care required. Prior experience in long-term care required. Unrestricted driver's and reliable transportation. Ability to travel to local facilities within the area.
Duties and Responsibilities:
1. Generate admission activity to meet and exceed census budget. 2. The admission nurse will act as a representative for a facility and assigned hospitals and referral sources for families. The person will act as a representative for the facility and for assigned hospitals and referral sources for families. This person will be responsible for building relationships with hospitals, which generate patient referrals and admissions. This person will assess clinical needs of potential / existing residents to assure the facility has the clinical capability to treat the resident. Must have the ability to establish good working relationships with social workers, nurses, Physicians, and case managers in assigned hospitals and other medical facilities as appropriate Represents the facility when interacting with referral sources, business leaders, physicians, and the community when marketing. Develop strong relationships with key referral sources and hospital personnel to identify new opportunities for alignment, partnering, and growth. Assist in planning and organizing marketing strategies and programs designed to meet census and payor mix goals. 3. Managing the admission process. Assure that the admission process is seamless and positive, including but not limited to identifying all necessary device/equipment and immediate medical needs necessary to ensure a smooth transition to WWM. Initial screening of potential residents. Tours with families of potential admission and answer all questions. Gathering necessary paperwork and notify necessary departments. Assign room numbers to residents in consultation with nursing department and administration Greet and escort residents to assigned room on unit. On admission take pictures of new residents and load to PCC. Enter bed offers, all standing admission orders and weights into PCC. Upload all hospital records into PCC. Enter admitting diagnosis and diagnosis codes from therapy into PCC. Initial care plan assessment. Obtaining preauthorization for admission. Complete the PASRR preadmission level one screening on all new admissions. 4. Financial and Regulatory Compliance Follow admission, transfer and discharge policies and procedures Understanding of insurances and managed care plans to appropriately place residents. Insure accurate completion of admission paperwork. Establish initial admission diagnosis and plan of care. Coordinator of Path Meeting within 5 to 7 days of admission Resident insurance updates as requested by Geisinger, Blue Cross, Aetna, or other primary insurances. Obtain end of Medicare signatures from residents or family when insurance ends their services.
Appeals:
complete the appeal process as required. Complete cut letters. 5. Discharges Open and complete section A on form Discharge Instructions in PCC Discharge education to ensure safe transition to home focusing on the prevention of readmission Prepare discharge packet with IDT assistance. Set up Home Health services with agency of choice and additional community resources. Follow-up appointments. Post discharge survey. 6. Additional Duties Work on completing the MDS interview and complete sections

Specific Requirements:
Knowledge of healthcare regulations and policies. Strong written and verbal communication skills. Participate in QAPI through data management. Excellent organizational skills with a detail orientation towards documentation. Superior follow-up skills and ability to organize and prioritize numerous tasks. Professional telephone skills with the ability to handle referral calls effectively and efficiently, meeting the needs of the referral sources and families. Understand and communicate critical and/or strategic changes to hospital functions, relationships, programs, and opportunities. Ability to work with diverse populations.
Physical Requirements:
Must be able to sit, stand, bend, lift and move intermittently throughout the workday. Must be able to lift a minimum of 50 pounds. Must be able to work independently. Must have the ability to thrive in a dynamic high stake environment and deliver results in tight deadlines.

$35.00 - $45.00 per hour
