Job Summary:
These positions plays a vital role in supporting individuals and families by coordinating services, maintaining documentation, and ensuring compliance with program requirements. In this role, you will provide comprehensive support to individuals facing behavioral health challenges, addiction issues, and social service needs. You will coordinate care plans, facilitate access to resources, and promote positive outcomes through evidence-based interventions. The ideal candidate will possess strong skills in crisis management, motivational interviewing, and workforce development to empower clients toward recovery and stability. Responsibilities Conduct thorough assessments of clients' needs related to behavioral health, addiction, and social services. Develop individualized care plans that incorporate evidence-based strategies such as motivational interviewing and crisis intervention. Coordinate with healthcare providers, community organizations, and support networks to ensure clients receive appropriate services. Provide ongoing case management support, including monitoring progress and adjusting care plans as necessary. Facilitate access to resources related to workforce development, housing, employment, and other social determinants of health. Maintain accurate documentation of client interactions, service plans, and progress notes in compliance with agency policies. Offer crisis management and immediate intervention when clients are experiencing acute behavioral or mental health emergencies. Advocate for clients' needs within the community and ensure compliance with Section 8 housing programs and other relevant policies. Qualifications Bachelor's degree in Social Work, Behavioral Health, Psychology, or a related field; Master's degree preferred. Proven experience in case management within behavioral health or social services settings. Knowledge of addiction counseling techniques and behavioral health interventions. Strong skills in motivational interviewing and crisis management strategies. Familiarity with Section 8 housing policies and workforce development initiatives is highly desirable. Excellent communication and interpersonal skills with the ability to build rapport with diverse populations. Ability to work independently while collaborating effectively within a multidisciplinary team. Certification or training in behavioral health or social work is a plus.
