Job Description

SUMMARY OF JOB
: Provide services to developmentally and mentally disabled customers while enhancing customer's community functioning and increasing independence. Support community placement and independence of customers who may otherwise end up in more restrictive settings due to severity of illness or disability.
Essential Job Functions:
Effective verbal and written communication skills with both internal and external contacts. Ability to relate, empathize, and show compassion for a diverse population and a variety of personalities. Able to work within a team environment. Able to problem solve. Protect patient heath information. Maintain driver's eligibility and comply with Vehicle operations and customer transportation policy. Follows Pivotal Policies including Code of Ethics. Proficient with a variety of computer programs related and required to perform duties of the position. Maintains education/training requirements set by Pivotal/SWMBH by August 1 each year.
Specific Job Functions:
Knowledge and experience working with individuals with intellectual disabilities, including those with co-occurring mental health conditions. Support person centered planning. Supports a trauma informed culture to individuals in their recovery process. Knowledge of the Diagnostic Statistical Manual and Evidenced-Based practices. Actively support and practice person centered planning, share trauma informed culture and recovery-based practices. Knowledge of adult and children's psychiatric disorders. Knowledge of basic counseling and psychotherapy techniques. Understanding of ethical implications to the customers/therapist relationship. Requires strong communications skills. Ability to conduct assessments and identify member's priorities and needs.
Physical Requirements/Working Conditions:
Requires ability to lift twenty-five pounds. Prolonged sitting and standing as appropriate. Stooping, kneeling, crouching, or bending as appropriate. Requires coordination of hands and/or eyes/hand/foot. Office environment with noise from computers, printers, and telephones. Use of computer for more than 30 minutes at a time. Possible eyestrain from extended periods of viewing computer screen. May be exposed to bloodborne pathogens, infectious diseases, and parasites. Travel is required for this position. Job Duties Ensure that person centered plans are completed in a timely manner upon entry to enhanced services and updated at least annually thereafter for primary caseload customers Monitor progress, or lack thereof, toward treatment goals Ensure timely completion of reviews for primary caseload customers and make modifications to treatments plans as needed Follows principles of self-determination Provide crisis response as needed to ensure that emergent situations are resolved in a timely and appropriate manner with may include referral to an inpatient or alternative placement Ensure that benefits (e.g., Medicaid and Social Security, spend downs) are secured and maintained Monitors and coordinates services with other agencies, providers, and significant people (e.g., medical professionals, law enforcement, courts, landlord, AFC provider, family members, guardians, representative payee, day program providers) Manage alternative treatment orders form probate court. Including clinically appropriate and effective use of an order's authority and ensuring orders remain in force when needed Ensure that all paperwork related to AFC placement is completed in a timely and accurate manner. This includes AFC Assessment Plan, 3803, model payment and DSS/SSA Referral form. Ensure all paper related to specialized or crisis residential placement is completed in a timely and accurate manner. Provides advocacy to encourage the customer's inclusion in community life and nondiscriminatory practices with respect to developmental disabilities Coordinates and monitors all aspects of support waivers services (e.g., assisting family with application, developing plan of service according to State guidelines and IPOS principles Provides direct customer contact according to treatment plan and customer needs. Assess, mental status and psychosocial functioning of customers Assists in all areas of life to ensure that basic needs are met and that the customers are living in the least restrictive setting. This may include assistance with budgeting, finding, and maintaining housing, grocery shopping, laundry, transportation, and recreation. Monitors medication, usage, compliance, and side effects: coordinate these activates with the prescribing professional Provides/ assist with guardianship paperwork in accordance with the Mental Health Code Helps with Supported employment (e.g., assisting family with application, developing plan of service) Follows Company's Code of Ethics, Corporate compliance guidelines, standards of business conduct and company policies, (such as attendance, punctuality dress code and work ethic) Protects client's rights by maintaining confidentially and reporting complaints, instances of suspected abuse, neglect, or misappropriation of property. Attends all required trainings and staff meetings (e.g., team, clinical and in-services as specific to job responsibilities) Submits all non-clinical documents in a timely and accurate manner (time sheets, date forms, training logs, conference request forms, payroll, and personnel forms) Completes annual training log by August 1. Reads/answers e-mails, listens/responds to voice mail within 24 hours of receiving the messages, unless on leave Other job-related duties as assigned. These duties may include any assignment that provides for the efficient function of the department.
