Videos Duties Help The Care in the Community (CITC) Nurse Practitioner is responsible for identifying MISSION Act eligibility and guidelines, understanding use of standard episodes of care (SEOCs), reviewing, and summarizing medical documention, determining approprieness of request for services, entering new requests for services as needed, and continuion of care as required. Incumbent is responsible to perform both clinical and administrive duties, ensuring quality pient care and approprieness/timeliness of services. Incumbent works directly with CITC staff in the delivery of community care services to our Veteran populion. Incumbent assists CITC to fill in gaps th Primary Care and Specialty Care providers the VA Maryland Health Care System (VAMHCS) leave in the ordering and review of care to be delivered by community providers. Incumbent functions as an independent member of the interdisciplinary team to ensure th the Veteran receives all care and services as required by their healthcare needs. The incumbent independently manages consults and referrals for pients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. The NP collabores with pients, families, and the health care team to develop and implement an individualized plan to achieve optimal populion health and disease management and will assess and adjust the plan of care as needed to achieve optimal pient outcomes. The NP is responsible for ordering and interpreting labs, tests, and diagnostic studies, and prescribing pharmacologic and non-pharmacologic interventions. The incumbent provides educion to pients and their families. The NP provides case management for clinical review of consults and referrals to include specialty.

Basic Requirements Educion:
A post-master's certifice or master's or doctoral degree as a Nurse Practitioner. (Applicants pending the completion of educional requirements may be referred and tentively selected but may not be hired until the educion requirements are met.) Must be a gradue of a school of professional nursing approved by the approprie accrediting agency and accredited by one of the following accrediting bodies the time the program was completed by the applicant: The Accredition Commission for Educion in Nursing ( ACEN ) or The Commission on Collegie Nursing Educion ( CCNE ).
NOTE:
Foreign Educion. Gradues of foreign schools of professional nursing programs, the nurse practitioner degree equivalency must be valided through an Agency recognized degree equivalency evaluion agency, such as the Internional Consultants of Delaware ( ICD ).
