The Advanced Practice Pharmacist provides direct ambulatory patient care as a credentialed and privileged non-physician medical provider with clinical supervision from the Medical Director under the authority of a Collaborative Practice Agreement (CPA). The Advanced Practice Pharmacist is a key member of the medical team and performs patient assessments, initiates and manages medication therapy, monitors medication safety and effectiveness, provides patient education, and collaborates with other providers.ESSENTIAL FUNCTIONS
Provides direct patient care for all NHBP clinics. Evaluates and manages patient diseases and health conditions: Collects and reviews patient medical and medication histories. Performs patient assessment and assesses vital signs. Orders and interprets laboratory values. Orders referrals to internal or outside specialist providers when indicated. Initiates, adjusts, modifies, or discontinues drug therapy within the scope of the CPA and area of specialization. Designs and implements medication, monitoring, and adherence plans. Provides disease state and medication education. Provides care coordination and transition of care services. Documents patient encounters, consultations, and prescriber interactions in electronic medical record according to all local, Tribal, State, and Federal policies and regulations. Works in collaboration with all available resources, including medical, behavioral health, community health, and other external sources, to provide optimal mediation management for patients within defined practice area. Evaluates the quality outcomes of patient care and implements new processes for continuous improvement. Participates in quality initiatives, projects, committees, policy development, and formulary management strategies. Oversees pharmacy program within their area of specialization, ensuring competencies for pharmacists, up-to-date protocols and drug therapy guidelines, and quality assurance in defined area of practice. Collaborates with the Pharmacy Director to monitor the quality and safety of the offsite pharmacy distribution processes at the Pine Creek and Grand Rapids locations. Provides staffing coverage in the retail pharmacy as needed for patient care and operations. Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards. Complies with accepted standards of care, confidentiality and privacy laws, and NHBP policies and procedures. Participates in clinical team conferences, quality assurance, planning, and other types of meetings or activities designed to review, improve, enhance or expand services. Maintains professional and technical knowledge by attending formal continuing medical education programs, participating in performance improvement activities, and doing self-study activities. Maintains licensure in the State of Michigan by attending approved continuing education activities.MINIMUM QUALIFICATIONS
Doctor of Pharmacy (PharmD) or, if earned before 2003, Bachelor of Science in Pharmacy from an accredited program. State Board Licensed in the State of Michigan. Completion of a PGY1 Pharmacy Practice Residency or a minimum of one (1) year of clinically integrated patient facing practice. Complete and pass a comprehensive background investigation and drug screening in accordance with NHBP standards. Experience working with highly confidential and sensitive information and both preserving and maintaining confidential and sensitive information, which includes following HIPAA requirements. Work or education experience with written and oral communication.OTHER SKILLS AND ABILITIES
Thorough knowledge of pharmacy and medical professional standards. Knowledge and understanding of relevant physiology, pathophysiology, and disease states pertinent to scope of practice defined in collaborative practice agreement. Possess a broad professional knowledge in the field of clinical pharmacy and aspects of the total pharmacy operations. Basic knowledge of medicine, chemistry, pharmacokinetics, pharmacology, microbiology, pathology and toxicology to independently perform professional assignments. Basic technical proficiency in digital communication, internet searching, and generating documents electronically. Knowledge of proper drug and medication storage to ensure perseveration and security. Knowledge of potential side effects of certain drugs or medications to properly advise and counsel patients. Clinical assessment of patients and their drug regimens, the development of nonpharmacological and pharmacological therapeutic plans. Understand, gain knowledge and appreciate the differences with the Native American culture and customs. Develop a comprehensive understanding of Trauma Informed Care and apply to daily functions. Understanding of, or willingness to learn, and ability to support programs and services that are culturally specific, cultural in nature, and culturally focused. Work creatively with other health care professionals from a variety of disciplines to achieve maximal program effectiveness and patient benefit. Comprehensive understanding of, or willingness to develop a comprehensive understanding of integrated care models as they apply to daily functions within an integrated health facility. Ability to competently utilize an electronic medical record for documentation of patient care. Ability to comprehend and abide by Tribal, State, Federal and other relevant regulations and policies. Ability to be self-directed and be an excellent problem solver. Ability to work with constant interruptions and organize/prioritize duties. Ability to foster a positive work environment.PREFERRED QUALIFICATIONS PGY1
Pharmacy Practice Residency and PGY2 Ambulatory Care Residency and history of providing top-of-license clinical services under a Collaborative Practice Agreement. Board Certified Ambulatory Care Pharmacist (BCACP) Board Certified Pharmacotherapy Specialist (BCPS) considered. 3 years' experience providing advanced practice pharmacy services in a Tribal community.SECURITY SENSITIVE
