Job Description

Family Nurse Practitioner or Physician Assistant Position Summary:
The Advanced Practice Provider is a key member of the primary care team, delivering high-quality, evidence-based care to patients of all ages in an outpatient clinic setting. The APP will independently manage common acute and chronic conditions, promote preventive care, and collaborate with physicians and support staff to ensure a patient-centered experience.
Essential Duties and Responsibilities:
Provide comprehensive primary care, including history, physical examinations, diagnoses, and treatment plans Manage chronic illnesses such as hypertension, diabetes, asthma, and hyperlipidemia Evaluate and treat common acute illnesses (e.g., respiratory infections, urinary tract infections, minor injuries) Order and interpret diagnostic studies (lab work, imaging, etc.) Prescribe medications and other treatments in accordance with applicable laws and scope of practice Educate patients on disease prevention, health maintenance, and self-care Coordinate referrals to specialists and follow-up care as needed Document all patient encounters thoroughly and timely in the Electronic Health Record (EHR) Participate in clinic quality improvement initiatives and population health programs Maintain collaborative practice agreements and adhere to clinic policies, procedures, and protocols
Education & Licensure:
Master's degree or higher from an accredited Nurse Practitioner or Physician Assistant program National board certification (AANP, ANCC, NCCPA, etc.) Active and unrestricted state license (NP or PA) DEA license with prescriptive authority BLS certification (ACLS preferred but not required)
Experience:
1-2 years of experience in primary care or family medicine preferred New graduates with strong clinical training may be considered
Skills:
Strong clinical assessment, diagnostic, and problem-solving skills Effective communication and patient education abilities Team-oriented with a patient-first attitude Comfortable with EHR systems (e.g., Epic)
Working Conditions:
Outpatient clinic setting, typically Monday-Friday daytime hours No hospital rounds, no inpatient call Requires regular use of computers, walking between exam rooms, and occasional lifting of medical supplies or small equipment

$100,000.00 - $125,000.00 per year
