Advanced Practice Registered Nurse (APRN)

Job Summary We are seeking a dedicated and skilled autonomous, board-certified Nurse Practitioner (Advanced Practice Registered Nurse, APRN) to join our healthcare team. The ideal candidate will provide high-quality patient care in a Primary Care Setting, focusing on individuals for wellness and prevention. This role involves performing physical examinations, assessing patients, diagnosing and treating common illnesses, prescribing medications, and educating patients on health promotion and disease prevention. Must be familiar with documentation in electronic health record. Experience in Functional Medicine is highly desirable. Responsibilities Conduct comprehensive physical examinations and assessments of patients. Develop and implement individualized care plans based on patient needs. Provide health coaching to empower patients in managing their health conditions. Manage cases for patients, ensuring appropriate support and resources. Administer treatments as indicated. Collaborate with interdisciplinary teams to coordinate patient care effectively. Maintain accurate and up-to-date patient records using EHR. Provide education on disease prevention and management, promoting overall and optimal wellness. Qualifications Current licensure as an Advanced Practice Registered Nurse in the state of practice. 2 - 3 years of experience as APRN mandatory requirement Previous experience in Functional Health is required (minimum of 6 months to 1 year) Strong knowledge of health coaching and lifestyle principles. Familiarity with electronic health record systems. Proficient in aseptic techniques and infection control measures.
