The Medication History Associate I has the responsibility of obtaining an accurate and complete account of the patient's current medications.

Required Education:

High School Diploma, GED, or Equivalent.

Required Certifications:

AHA BLS Provider. Collects the medication history for the admitted patient for the Admitting Physician to review and reconcile medications Utilizes AllScripts to review history of patient's home medication and allergies Utilizes McKesson to place the patient's medications and allergies into the EMR accurately Works collaboratively with the Admitting Physician, ED Physician, ED Nurses, and Inpatient Nurses to ensure an accurate account of the patient's current medications Researches patient's current home medications by working collaboratively with retail pharmacies, nursing homes, families, hospitals, or doctor's office May perform other duties as assigned or requested and job specification can be modified or updated at any time
