Job Description

The Supervisor, with the Outreach Coordinators, provides preliminary screenings, recommendations about critical service needs, and necessary interventions and referrals Works closely with psychiatric staff, nursing personnel, Peer Support Staff, and Community Support Staff, and other community-based services to coordinate care for individuals served. Provides an ongoing coordination and monitoring function, following the interventions and goals as specified in the individualized action plan. Works with the individual served, their families, peer and community supports, and other providers, DMH, etc. to maximize the chances of effectively engaging the individual served. Oversees all aspects of medication treatment and administration, including preparing Health Care Provider Orders, Medication Administration Records, Current Medication Lists, Weekly MAP Checklists, Monthly MAP Checklists, and Medication Occurrence Reports and ensures corrective action required by a MAP Audit occurs. Provides crisis intervention and emergency services as needed, including serving as a member of the area on call team. Reviews portions of the electronic health record prepared by supervisees in a thorough and timely manner. Ensures communication between staff, the program, and outside agencies including health care providers and pharmacies, the program, and families. Supervises the Integrated Team Coordinators. Participates in the hiring and onboarding process as assigned. Addresses disciplinary issues as needed and completes introductory and annual performance evaluations. Oversees staffing schedules to ensure adequate coverage to meet individual and program needs. Who You Are Associate's Degree in a Human Services field from an accredited university and one year of experience working with the target population required. Supervisory experience required. Individuals must be committed to empowerment and growth, human rights, and community integration of individual served and be familiar with principles of recovery, evidence-based practices used in ACCS, trauma informed treatment. Valid driver's license and a reliable vehicle available for all working hours required. We Hire for Purpose!
