Responsibilities:

Develop and implement Individual Service Plans, based on assessments and observations. Responsible for 24 hour a day Group Home with rotating on-call and scheduling. Supervise, plan, assign, and evaluate or monitor programming and the work of staff responsible for the daily care of individuals. Work closely with therapists, service coordinator, administrations, and parents/guardians to maintain communication and provide feedback, standardized procedures, expedite support plan implementation and work-flow, and to improve the efficiency of all care/support providers. Performs various assessments; chart reviews; documentation, mealtime and environmental monitoring.

Qualifications:

BS in Social Work, Social Services, Nursing, Special Education, or related field from an accredited four (4) year college or university. 2 or more years' experience in Program Management in a similar setting Proficient in MS Office suites and various industry software programs Valid Missouri License with an insurabcle driving record Learn more about what it's like to be a member of the Pony Bird team at www.ponybird.org/news Pony Bird, Inc is an Equal Opportunity Employer EOE/M/F/D/V/SO
