  • While performing the duties of this job, the employee will be required to work in a variety of settings and environments and has an occasional exposure risk to adverse weather conditions.
  • The noise level in the work environment is usually light to moderate.
  • In the event that unanticipated aggressive behavior manifests from individuals served and de-escalation techniques fail, physical crisis intervention may be required.
  • The work hours of this job are 8 hours per day, 5 days per week but it is also expected for employee to be available during off hours to answer work related phone calls, emails, or text messages and, if need be, be physically available to go on site Physical Demands

Primary Purpose of Job To supervise the House Managers in multiple community homes by providing oversight of all daily operations in each home; ensuring health and safety of individuals served, adhering to the 6400 ICF/MR regulations and the policies and procedures of Divine Providence Village. Essential Job Functions Supervises approximately 3-4 house Managers Evaluates performance of House Managers, providing corrective action and recommending termination as needed. Monitors schedule of HM and staff the schedule for House Manager Monitors the direct care schedules for each home, to ensure that prescribed levels of supervision and budgetary constraints are observed. Monitors requests for PTO, Bereavement Leave as well as requests Unprotected Leave to ensure adherence to DGV policies and procedures, working with the Residential Management team and Scheduler to distribute PTO time in fair and equitable manner. Reviews time and attendance records daily and ensures accurate punch data in conjunction with the Scheduler. Working closely with the Program Directors, monitors the implementation of all Individual Program Plans and Behavior Support Plans Monitors documentation in each home for compliance including but not limited to: goal documentation, behavior tracking, progress notes, community outing logs, medication administration records Ensures proper follow up to individual incidents in accordance with the Incident Management Bulletin and the Incident Management Policy. Other Job Functions Adheres to the Policy and Procedures of Divine Guanella Village and ensures that all residential employees comply with the policies and procedures of Divine Providence Village as well as the requirements set forth by the Department of Public Welfare. Maintains compliance with the Training Policy inclusive of all mandatory trainings; demonstrates competence and acquires certification in CPR/FA Works collaboratively with other departments to communicate medical, social, and behavioral needs of individuals served, in a timely fashion, as well as the daily operational needs of the organization. Maintains professional communication with families, guardians, and advocates of the residents and all stakeholders.
Medication Administration responsibilities:
    Practicum Observer:
    • Successfully complete and pass the DPW Medication Administration Practicum Observer course, practice activities, examination and initial observations.
    • Maintain DPW Practicum Observer certification by completing all requirements on an annual basis within the designated time period.
    • Oversee, monitor and supervise the administration and documentation of medication by certified DPW Medication Administration employees.
    • Follow process and procedures to identify and report medication errors and concerns.
    Med Certified Staff:
    • Successfully complete and pass the DPW Medication Administration course, practice activities, examination and initial observations.
    • Maintain DPW Medication Administration certification by completing all requirements on a semi-annual and annual basis within the designated time period.
    • Correctly and safely administer medications in the program area.
    • Adhere to procedures of documenting medication administration in the program area.
    • Follow process and procedures to identify and report medication errors.
    Responsible for other duties as assigned by the Director of Operations and Administrator
    KNOWLEDGE/ SKILLS/ ABILITIES

    Education/Experience:
    Bachelor's degree from an accredited four-year college or university One year related experience preferred or Associates degree and 5 years related experience.
    Training:
    Ability to read and write competently in English. Ability to carry out instructions furnished in written, oral, or diagram form. Ability to problem solve and provide solutions when challenges arise unexpectedly. Working knowledge of
    MS Word, Excel, Outlook Certificates and Licenses:
    Valid (state of residence) Driver's License and driving approval from the Archdiocese of Philadelphia is required. Must attend and acquire certification in the DPW Certified Investigator Training (within one year of hire date, as training schedule permits) Qualifications

    • Must be able to get and maintain acceptable background clearances - FBI, PA State Police Criminal History, and PA Department of Human Services Child Abuse
