Residential Coordinator-Full Time Horizon House - 3.4 Philadelphia, PA Job Details Full-time 1 day ago Benefits Commuter assistance Paid holidays Health insurance Dental insurance Tuition reimbursement Paid time off Opportunities for advancement Retirement plan Qualifications Crisis intervention Shift management Mid-level Social Sciences Driver's License Supervising experience Driving Working with individuals with mental health conditions Patient progress monitoring Progress documentation Associate's degree 2 years Communication skills Rehabilitation and recovery (patient care) Full Job Description We are seeking a Residential Coordinator for our Behavioral Health Services Division to coordinate and provide guidance, counseling, and encouragement to clients as they transition from the mental health system to a more independent environment; assist the Manager with daily site operations; and supervise direct service staff in implementing and documenting client progress, in accordance with individualized rehabilitation service plans. Other responsibilities include; Provide support and ensure the delivery of appropriate services; review client accessibility to services. Reinforce implementation of the individual rehabilitation service plans; incorporate modifications and updates, as directed. Responsible for the operation of the site during assigned shift and/or in the absence of the Manager.
Qualifications :
Associate's degree in the Social Sciences, healthcare, or a related field and 2-3 years of relevant experience in the MH/MR field; or any combination of experience, education, or training that would provide the level of knowledge, skill, and ability required. Good communication skills, both oral and written. Ability to work independently and as part of a team. Some knowledge of mental health systems and supports. Prior supervisory experience is helpful.Scope:
Requires the ability to lift, carry, fold, kneel, reach, stack, stoop, and bend, which may include direct intervention with residents. Possess manual dexterity and fine motor skills. May be required to possess a valid driver's license and an acceptable driving record (contingent upon facility location and client and program needs). May require crisis intervention in emergencies. May be required to report to work during emergencies, including inclement weather.
