2 days remote, 2 days in office; schedule may vary based on needs) Reports to :

Key Responsibilites Lead the design, execution and evaluation of the community based programs Oversee quality control initiatives, including CQI, Management Information Systems and Safety Committee Processes Documentation, billing and reporting Identify new service opportunities with Heart to Heart service area Provide staff training and ensure compliance with agency, state and federal requirements Conduct site visits and address urgent program or individual needs through 24/7 on call coverage Partner with Recruitment team to ensure effective program staffing Perform other duties as assigned by Director to support the success of the program and services Must be at least 21 years of age Two more years experience working with individuals with intellectual and developmental disabilities Bachelors degree (preferred) High School Diploma (required) Must pass all criminal background checks and drug screening Valid NJ Driver's License and reliable vehicle (mileage reimbursed for work related use) Strong written and verbal communication skills Knowledge of best practices in program development and community supports Knowledge of Therap and HHAExchange preferred Proficient in Microsoft Office Strong organizational skills and ability to manage multiple priorities Excellent interpersonal skills to work effectively with diverse groups The Community Supports Manager oversees the development, implementation and quality of non-licensed programs at Heart to Heart. This role ensures services meet the needs of individuals with intellectual and developmental disabilities, comply with state and federal guidelines and aligns with our mission. The CSM works closely with internal teams and external partners to maintain program excellence, foster community connections and ensure the individuals satisfaction with provided services.
