Contractor for Home Modification Class A, B or C Charity Care Group The Foundation 100 Bosley Avenue, Suffolk, VA 23434
Project Overview:
Collaborative Home Modification Initiative for Aging in Place Charity Care Group, in partnership with the U.S. Department of Housing and Urban Development's Older Adults Home Modification Program (OAHMP), is launching a comprehensive initiative to enable low-income elderly homeowners in Norfolk, Virginia to age in place safely and comfortably.
This collaborative project aims to:
Implement low-cost, high-impact home modifications to reduce fall risks, improve safety, and increase accessibility for seniors aged 62 and older. Modifications may include installing grab bars, railings, lever-handled doorknobs and faucets, temporary ramps, and adaptive equipment. Utilize a person-centered approach, incorporating the expertise of licensed Occupational Therapists (OTs) to tailor modifications to each client's specific goals and needs. OTs will evaluate clients' functional abilities and home environments to optimize independence. Serve approximately 50 low-income elderly homeowners over a 3-year period, focusing on those unable to perform modifications themselves. Coordinate with local community organizations, healthcare providers, and senior services to identify and reach eligible participants. Provide education and resources on fall prevention, home safety, and aging in place strategies to program participants and the broader community. Leverage Charity Care Group's existing relationships and utilize Certified Aging-in-Place Specialists (CAPS) to maximize program impact and efficiency. Adhere to HUD's guidelines, including the $5,000 per unit cap for modifications, while ensuring comprehensive support for each participant. Contribute to the growing body of research demonstrating the positive impact of home modifications on fall prevention and disability reduction among community-dwelling elderly. Insurance Requirements:
General Liability Insurance:
Minimum $1,000,000 per occurrence and $2,000,000 aggregate
Workers' Compensation Insurance:
As required by Virginia state law
Professional Liability Insurance:
Minimum $1,000,000 per claim
Automobile Liability Insurance:
Minimum $500,000 combined single limit

Budget Guidelines for OAHMP Home Modification RFP Maximum Budget per
Home:
As per HUD guidelines, the maximum budget for home modifications is $5,000 per unit. This $5,000 cap includes all costs related to physical modifications, including labor, materials, and equipment. Contractors must strive to maximize the impact of modifications within this budget constraint. In exceptional cases where modifications may exceed the $5,000 cap, contractors must provide detailed justification for review and potential pre- approval by HUD.

Contractors must provide a detailed, itemized cost breakdown for each proposed modification.
This breakdown should include:
Labor Costs:
Hourly rates for different types of labor (e.g., carpenter, electrician, plumber) Estimated hours for each type of labor per modification Total labor cost per modification
Materials:
Itemized list of all materials needed for each modification Unit cost for each material Quantity of each material required Total material cost per modification
Equipment:

Additional Budget Considerations:

Payment Schedule:
Provide a proposed payment schedule aligned with project milestones. Final payment will be contingent upon satisfactory completion of work and approval by the Occupational Therapist or CAPS certified professional.
Cost Reporting:
Contractors must be prepared to provide detailed cost reports for each completed modification project. These reports will be used for program evaluation and reporting to HUD.
