Full Time Social Worker - Payson, AZ Connected Health Care is currently seeking a passionate and dedicated Full Time Social Worker to join the team at our partnered hospital in Payson, AZ. Provide comprehensive social work services to patients and their families, including psychosocial assessments, counseling, resource referrals, and discharge planning. Collaborate with interdisciplinary teams to develop and implement individualized care plans. Advocate for patients' rights and coordinate community resources to ensure continuity of care. Maintain accurate and thorough documentation in compliance with regulatory standards and hospital policies. Participate in ongoing professional development and training opportunities.Education and Certification Requirements:
Master's degree in Social Work from an accredited institution. Current licensure as a Social Worker in the state of Arizona. Minimum of 2 years of clinical social work experience preferred. BLS certification required.About Payson, AZ:
