Job Description
Full-Time Hospital Social Worker in Bishop, CA, delivering emotional, psychological, and social support to patients and families. You will collaborate with a multidisciplinary team to address complex healthcare challenges and facilitate holistic care.
Key Responsibilities:
Conduct psychosocial assessments and evaluations for patients and their families. Provide counseling and support for patients dealing with medical, emotional, or financial challenges. Assist with care coordination and collaborate with medical staff to develop discharge plans and ensure a smooth transition of care. Provide referrals to community resources such as housing, mental health services, and financial assistance. Support patients and families in navigating complex healthcare systems and advocate for patient rights. Offer crisis intervention services to patients experiencing urgent medical, emotional, or situational crises. Document all patient interactions and social work interventions in accordance with hospital policies and regulatory requirements. Participate in case reviews and care planning meetings with other healthcare professionals. Ensure compliance with state and federal regulations related to healthcare services.Qualifications:
Bachelor's degree in Social Work or related field required. Licensed Clinical Social Worker (LCSW) required. Valid California state licensure as a Clinical Social Worker required.Preferred Skills:
Master's degree in Social Work (MSW) from an accredited university preferred. Minimum of two years of clinical experience in a hospital or healthcare setting preferred. Basic Life Support (BLS) certification preferred. Experience in hospital-based or medical social work. Knowledge of community resources and services available in Bishop and surrounding areas. Strong interpersonal skills and ability to collaborate with healthcare teams. Crisis intervention experience and ability to work in high-pressure situations.
